Patient Waitlist Registration Form

The waitlist is open for the following groups in order of priority:

1. Children aged 0-18 years who have a parent that is already a patient of the clinic.

2. Adults aged 18-49 who have a sibling or spouse who is already a patient of the clinic.

3. Adults aged 50+ who have a child, sibling, or spouse who is already a patient of the clinic.

We are not currently accepting patients who are outside of these groups.

We are not accepting patient who already have a family doctor.

We are not accepting RAMQ patients.

We will consider patients who have a family doctor retiring in the next 12 months.

If registering more than one patient, please complete one form for each patient.

Applicant Eligibility Confirmation

Once you have submitted your application to the waitlist, it may take up to 12 months for your submission to be processed. If you submit a misleading application, for example, you apply because your cousin or friend is a patient or you apply and you already have a family doctor, you will be banned from applying in the future.

Due to the high volume of mesages regarding the waitlist, any emails about accepting patients outside of these groups will be discarded.
